Virgin coconut oil (VCO) consists highly saturated and medium chain fatty acid, which
potentially used for total parenteral nutrition (TPN), by formulating it as microemulsion.
Factors affecting globule size and hemolytic activity are important issues in
microemulsion for TPN, which were studied in this current research. Factors evaluated
include impeller types, preparation size, VCO source variation, soy lecithin preparation,
and sterilization method. Hemolytic activity was studied using in vitro static and
dynamic method on VCO microemulsion, 2-1 admixture (dextrose 40% addition), and
3-1 admixture (dextrose 40%, and amino acid 10% addition). Globule size measured at
day 14 was 57.37 ± 13.22 nm (50 mL) and 70.40 ± 2.41 nm (100 mL) for
2-blade impeller; 403.17 ± 43.73 nm (50 mL), 65.40 ± 9.57 nm (100 mL) for
4-propeller impeller; 70.40 ± 2.41 nm (VCOI); 72.83 ± 2.31 nm (VCOII). Immediate
and overnight soaking of soy lecithin did not significantly (p>0.05) affect the globule
size. Steam sterilization increased the globule size (79.93 ± 23.77 nm) compared to
before sterilization (52.37 ± 4.04 nm), whereas filtration sterilization did not influence
the globule size (54.00 ± 5.72 nm). Increase of globule size due to steam sterilization
was still in acceptable microemulsion range (10-200 nm). Single VCO microemulsion
showed higher hemolytic activity (83.39%) compared to admixture preparations.
2-blade impeller was more suitable for small scale preparation (50-100 mL).
Steam sterilization is recommended for VCO microemulsion. VCO microemulsion
showed high hemolytic activity.
