Sektor konstruksi merupakan salah satu pilar utama dalam pembangunan ekonomi dunia, termasuk Indonesia. Namun, hingga saat ini angka kecelakaan kerja di sektor konstruksi masih tergolong tinggi. 60 hingga 70 persen kecelakaan kerja di berbagai sektor pekerjaan disebabkan oleh human error. Analisis yang sistematis terhadap human error dapat digunakan untuk mencegah terjadinya kecelakaan kerja. Oleh karena itu, analisis human error penting untuk dilaksanakan, khususnya berkaitan dengan keselamatan dan kesehatan kerja (K3) di industri konstruksi. Tujuan dari penelitian ini yaitu untuk mengetahui kelompok/ jenis pekerjaan dengan risiko paling besar, mengetahui jenis kecelakaan kerja dengan HEP (human error probability) tertinggi, mengetahui penyebab utama human error yang terjadi, dan mengetahui kondisi implementasi K3 perusahaan berdasarkan human error analysis yang dilakukan. Metode yang digunakan dalam penelitian ini yaitu PSF (performance shaping factor)–based risk assesment dan HEART (human error assessment and reduction technique). Data primer diperoleh melalui wawancara, observasi, dan dokumentasi. Data sekunder yang dibutuhkan yaitu data kecelakaan kerja dan prosedur kerja dari ahli K3 proyek. Berdasarkan metode PSF-based risk assessment, dapat diketahui bahwa jenis pekerjaan dengan risiko paling besar terdapat pada kelompok pekerjaan kolom dan balok praktis serta pekerjaan kanopi beton yaitu pekerjaan pemasangan besi tulangan dengan cara pengelasan ke kolom atau balok baja, dengan nilai risiko 24 (skala 24), yang berasal dari bahaya terjatuh dari ketinggian. Hasil analisis dengan metode HEART menunjukkan bahwa kecelakaan terjatuh dari ketinggian memiliki nilai HEP tertinggi (4,75) dibandingkan kecelakaan-kecelakaan lainnya. Penyebab utama terjadinya human error di proyek konstruksi asrama ITB gedung C dan D yaitu tidak adanya prosedur tertulis yang lengkap untuk disampaikan pada pekerja. Berdasarkan analisis yang dilakukan, diketahui bahwa perusahaan perlu melaksanakan perbaikan implementasi K3 yang dapat dilakukan secara sistem, dimulai dari pihak manajemen, pengawas K3, dan pekerja.

Construction sector is one of the main pillars in the economic development of the world, including Indonesia. However, the number of accidents in the construction sector is still relatively high until now. 60 to 70 percent of occupational accidents in many employment sectors are caused by human errors. A systematic analysis of the human error can be used to prevent accidents. Therefore, the analysis of human error is important to be implemented, particularly with regard to health and safety in the construction industry. The objectives of this study were to know the stages/ types of work with the greatest risk, to know the type of occupational accident with the highest HEP (human error probability), to know the main cause of human error occurred, as well as to know the implementation of health and safety in the project based on human error analysis performed. The methods used in this research were PSF (performance shaping factor)-based risk assessment and HEART (human error assessment and reduction technique). The primary data were obtained through interview, observation, and documentation. Secondary data needed were occupational accidents data and work procedures from the project health and safety expert. Based on the PSF-based risk assessment method, it was known that the type of work with the highest risk occurred in the installation of iron reinforcement by means of welding to steel columns or beams, from columns and beams work and concrete canopy work, with the risk value of 24 (of 24), which came from fall from height hazard. While the HEART method showed that the fall from height accident had the highest HEP value (4,75) among any other accidents. The main cause of the human error occurred in the project was the absence of complete written procedures for workers. Based on the analysis performed, it could be known that the company needed to improve the implementation of health and safety in the project, which could be done systematically, started from the employers, safety supervisor, and workers.
